How to Make a Referral to Cheshire Young Carers
f you are a family member, or if you know a child or young person at Wolverham Primary and Nursery School who you believe is a young carer and could benefit from their support, making a referral is a straightforward process. We encourage you to reach out so they can assess their needs and offer appropriate assistance. School are proactive in identifying children who take on this role but always welcome your input.
Here's how to make a referral:
1. Contact Them Directly: The easiest way to start is to get in touch with their friendly team. You can do this by:

o Website: Visit their website at www.cheshireyoungcarers.org and look for the "Referral" or "Contact Us" section.
2. Assessment Process: Once they receive your referral, a member of their team will get in touch with school to arrange an education assessment. This is a chance for them to learn more about the young person's unique circumstances and for them to understand how Cheshire Young Carers can help. This assessment is always conducted sensitively and with the young person's well-being at its heart.
